External Rehabilitation Specialist Application

The City of Maple Ridge welcomes professionally affiliated External Rehabilitation Specialists. All External Rehab Workers must be approved by the City of Maple Ridge to use the Maple Ridge Leisure Centre facility to work with clients.

Must Include:

  • $2 million-dollar general liability
  • School District 42 listed as additional insured
  • The City of Maple Ridge listed as additional insured

  • British Columbia Association of Kinesiologists (BCAK) 
  • Rehabilitation Assistance Certificate/Diploma
  • Occupational Therapist Certificate/Diploma/Degree
  • Physical Therapy Certificate/Diploma

  • I will wipe down all equipment and machinery before and after use
  • I will refrain from monopolizing any one piece of equipment and ensure all users of the facility have equitable access to machines in the Fitness Centre
  • I will take phone calls outside of the Fitness Centre
  • I will not solicit my personal business while inside the Maple Ridge Leisure Centre
  • I will practice safe conduct and always provide safe instruction of exercises
  • I will follow the City of Maple Ridge’s Code of Conduct at all times
  • I will, and my clients, will follow the admission practices set in place by the City of Maple Ridge for External Rehab Specialists
  • I will defer questions from the public to facility staff
  • I will notify facility staff of any defective equipment
  • I will report First Aid incidents to facility staff
  • Individuals such as case workers, practicum students and shadow trainers must receive approval prior to entering the Fitness Centre at the Maple Ridge Leisure Centre
  • External Rehabilitation Services is strictly for rehabilitation. This service is not for Personal Training. If it is observed that you are using this service for for-profit personal training, you will be approached by Facility Supervisors, and your pass may be revoked
